Human Error

Accidents are not usually caused by a single failure or mistake, but by the confluence of a whole series, or chain, of errors. When looking at the root cause of incidents, it is usually possible to trace the origins and development on the incident to a number of discrete events. Many of which can be traced to Human Error. Despite mechanisation and technical advancement, the marine industry remains a people’s industry.
